Appointments & Cancellations
For your convenience, appointments can be scheduled by calling 602.726.3301. As a courtesy to other patients, we ask that office visit cancellations be called in at least 24 hours in advance.
What to Bring to Your Appointment
You will need to bring the following items to your visit:
- Insurance card(s)
- Driver’s license or other photo identification
- List of your current medications and dosages
- Any pertinent medical records or test results
- Insurance Co-payment
Patient Forms
Using our patient portal, new patients can save time by completing new patient forms prior to their first visit. Returning patients can complete forms assigned to them conveniently online.
Privacy Policy
Your privacy is considered a priority at Bates Family Practice. The HIPAA Notice of Privacy Practices describes how medical information may be used and disclosed.
Health Insurance Plans
Check our list of Accepted Health Plans. In lieu of health insurance, we may also work with patients on affordable cash pay options.
